Review the Reason Before Choosing the Response
Denials can involve eligibility, authorization, enrollment, coding, documentation, coordination of benefits or payer processing. The same unpaid balance may require a very different action depending on the reason.
We review available remittance details, denial codes, prior submissions and relevant account notes. Where the reason is unclear, follow-up can seek clarification from the payer before the next step is selected.
Denial Management Activities
- Categorization of denials by payer, reason and account priority.
- Review of claim details and prior billing actions.
- Identification of missing records or information requiring practice input.
- Preparation of supported corrected claims where appropriate.
- Coordination of reconsideration or appeal submissions within the agreed scope.
- Tracking of response dates, submission evidence and payer outcomes.
- Documentation of final account disposition or remaining next steps.
- Reporting of recurring issues that may require an upstream change.
Appeal availability and deadlines vary. Each account must be reviewed against the relevant payer requirements and dates rather than assigned a universal filing rule.
Correct the Claim or Challenge the Decision
An administrative error may be addressed through the payer's corrected-claim process. A disputed decision may require reconsideration or an appeal supported by records. These routes should not be substituted for one another without checking the payer's instructions.
Native RCM helps organize the chosen route and the evidence supporting it. Clinical records come from the provider; our administrative work should not alter facts to create a different reimbursement outcome.
Connect Denials With Their Source
Resolving an individual balance is useful, but a repeated error can continue generating work. We identify patterns that may connect to patient registration, prior authorization, credentialing or coding.
The finding can be routed to the appropriate staff member with a description of the issue and the process change being considered. This helps your practice address the source of future denials.
Prioritize Accounts With a Defined Review Process
Account age, balance, payer deadline, supporting records and previous actions all influence priority. We discuss how the denial queue should be organized and which exceptions need administrator approval.
Denial support can be combined with AR follow-up and reporting so unresolved payer balances are reviewed within a shared account history.
Denial Management Questions
Can every denied claim be recovered
No. Some decisions may be correct or no longer eligible for further review. We investigate the available route and document the outcome without promising recovery.
Can you review a backlog of denials
Yes. A backlog assessment can identify account age, denial categories, available records and the actions still open under payer rules.
Will you tell us why the same denials keep happening
Recurring issue analysis can be included in the reporting scope. The aim is to show which patterns require attention in the workflow that produced the claims.
